Director of Patient Care

The Gloucester Mathews Care Clinic has provided compassionate health care to the uninsured and underinsured residents of Gloucester and Mathews counties since 1998. Our team of staff and volunteers are dedicated to providing excellence in patient care.  We are seeking a full-time Director of Patient Care, RN required. Working collaboratively with the Physician Practice Manager and Executive Director, the Director of Patient Care is responsible for planning, organizing, directing, coordinating and evaluating patient care services provided by the Gloucester Mathews Care Clinic. Bilingual Medication Assistance Caseworker

7/13/20 - Richmond, VA
The Medication Assistance Caseworker (MAC) assists patients in obtaining the medications prescribed by the clinical providers at CrossOver Healthcare Ministry. The MAC assists individuals in applying for and obtaining free prescription medicines from the brand name pharmaceutical companies’ Patient Assistance Programs (PAPs) and/or discounted generic medicines by using The Pharmacy Connection (TPC). The MAC will also assist patients in obtaining medicines by helping those newly eligible for Medicaid apply for coverage. The MAC will continue to assist patients not eligible for Medicaid in obtaining medicines via the PAPs. This includes submitting Medicaid denial letters for those PAPs that require one. The MAC also is responsible for submitting prior authorizations to insurance companies if needed.

ADAMS Compassionate Healthcare Network (ACHN) is a nonprofit free medical clinic that provides free primary and specialty care services for uninsured and low-income individuals in Northern Virginia. We provide a wide range of patient services including primary and eye care, preventative screenings, blood work, imaging, prescriptions, and referrals. Our integrated approach also includes hot-spotting, telehealth services, care coordination, food and housing security evaluations, and preventative health education. We are seeking a Nurse Practitioner (NP) who will primarily be responsible for patient care in the office and Telehealth setting. Case Manager and Outreach Advocate

Clinic Manager

The clinic manager is responsible for managing the day to day operations of the medical clinic and for managing the patient experience for a diverse safety net medical clinic. Using both clinical knowledge and knowledge of HB policies and procedures, the clinic manager works collaboratively with the medical director, staff, and volunteers to ensure efficient and effective care delivery. Responsible for functioning of practice including management of staff, workflow development, patient schedule management, financial compliance and oversight of inventory, patient, provider and staff satisfaction and safety compliance. Operationalizes procedures and implements performance improvement strategies within the practice. Performs all duties in a manner which promotes good teamwork and reflects the mission and values of inclusive and trauma informed care. Clinical Director

The Clinic seeks a Clinical Director, a part time position (approximately 25 hours/week) where the incumbent manages a growing Clinical program and ensures effective and compassionate and patient care management.  The position works closely with the Executive Director on program development, with the Patient Services Manager to coordinate care and ensures effective communication among providers and staff and with the NP Population Health Manager to ensure effective care across our unique patient panel.
